Privacy Policy – Effective 9th July 2018

CareAscend, LLC (“CAREASCEND”) is sensitive to the concerns that you have about the use of the information that you disclose to CAREASCEND via its web and/or mobile application (collectively, the “Application”).  Accordingly, this Privacy Policy (the “Policy”) sets forth CAREASCEND’s privacy practices and the possible uses of the information that it gathers through the Application.  CAREASCEND will not sell, share, or rent Your Information to others in ways different from those disclosed in this Policy.

Collection of Information

In connection with your use of the Application, CAREASCEND may request and/or collect certain personal information from you, which may include, without limitation, your name, email address, employer, and/or job title (“Your Information”). By entering Your Information via the Application or otherwise providing Your Information to CAREASCEND, you consent to CAREASCEND’s use of Your Information for purposes discussed herein.

Use of Information

CAREASCEND will use Your Information in administering and supporting the Application and in providing you access to appropriate functions of the Application.  CAREASCEND will use Your Information in order to contact you and provide you with information regarding new features, updates, and other issues relating to the Tool and/or Application.

CAREASCEND will use also Your Information to respond to requests you may make of CAREASCEND.  CAREASCEND may also refer to Your Information to better understand your needs and how CAREASCEND can improve the Tool and the Application.

Information Sharing

CAREASCEND may share Your Information with agents, contractors or partners in connection with services that these individuals or entities perform for, or with, CAREASCEND. CAREASCEND may, for example, provide Your Information to agents, contractors or partners for hosting the Application, data processing services, or for sending materials to users of the Application.

CAREASCEND reserves the right to share Your Information in response to duly authorized information requests of governmental authorities or where required by law. CAREASCEND may also disclose Your Information if necessary for fraud protection and credit risk reduction purposes, or in the good-faith belief that such action is necessary to protect and defend the rights or property of CAREASCEND or the users of the Application or to act under urgent circumstances to protect the safety of CAREASCEND or its employees or a member of the public.

Aggregation and Cookies

Any of Your Information that is aggregated in a non-personally identifiable manner may be disclosed to third parties and otherwise used by CAREASCEND as it deems appropriate.  In addition, CAREASCEND may collect information through the use of common information-gathering tools such as web beacons, cookies, and server logging.  For example, the Application may use a technology called a “cookie.”  A cookie is a piece of information that the webserver sends to your device when you access the Application.  When you return to the Application, it will detect whether you have one of the cookies on your device.  The cookies help provide additional functionality to the Application and help CAREASCEND analyze Application usage more accurately.  For instance, the Application may set a cookie that keeps you from needing to re-enter your username or password.

Push Notifications

You acknowledge that CAREASCEND may send push notifications to you through the Application.

Ads

Ads appearing in connection with the Application may be delivered to you by advertising partners, who may set cookies. These cookies allow the ad server to recognize your device each time they send you an online advertisement to compile information about you or others who use your device. This information allows ad networks to, among other things, deliver targeted advertisements that they believe will be of most interest to you. This Policy covers the use of cookies by CAREASCEND and does not cover the use of cookies by any advertisers or other third parties.

Business Transfer

CAREASCEND may provide Your Information to a third party in connection with the sale, assignment, or other transfer of the business of CAREASCEND to which the information relates, in which case CAREASCEND will require any such buyer to agree to treat Your Information in accordance with this Policy.

Effective Date/Notification of Changes

Please note that CAREASCEND may update this Policy from time to time, and you should check this page periodically for changes. If changes are made, CAREASCEND will notify users by stating the date the Privacy Policy was last modified.  This Policy was last modified on 9th July 2018.
